APPOINTMENT INFORMATION
YOUR FIRST APPOINTMENT
We take a gentle approach to your oral health here at Inform Dental.
During your first visit to our practice, we will ask you to complete a Medical Health Questionnaire. This questionnaire is designed to help us offer you the most appropriate treatment based upon your health and medical situation.
All information collected from you is kept strictly confidential and you can be sure that we will not disclose any of your personal data without your consent.
During your examination, we will discuss your concerns and address any key improvements that you would like to see. We will examine the soft tissue of the external and internal areas of your mouth, perform a periodontal assessment and assess your oral hygiene. Following this we will carefully explain your options for treatment; we can also give you an indication of the costs associated with the treatment you will require.
Necessary radiographs will be taken along with any other diagnostics that may help in us in the planning for your treatment.

PAYMENT PLANS – FOR BRACES
Inform Dental understands that orthodontics can be a financial burden on a family, so discuss our payment plan option with the staff prior to the commencement of your treatment.
We require a deposit to be paid either prior to or on the day your treatment starts. The balance is then divided into 12 equal monthly payments. Inform Dental currently uses “FFAPaysmart” for all payment plans. This is an interest free direct debit system, taking payments from either your nominated account or credit card. If using a credit card a surcharge will be added to each payment. Paysmart charges an initial set up fee which is added to your first monthly payment only, also a small monthly administration fee is added to each payment.
